SSM Healthcare (SSM) is committed to providing financial assistance to people who are without insurance, underinsured, ineligible for a government program, or otherwise unable to pay for medically necessary care. Our Financial Assistance Policy also is available in Spanish or by contacting a Customer Service Representative.

 

How is financial assistance determined?

Financial assistance is based on need and determined by Federal Poverty Levels, which includes income and number of family members.  Financial need does not consider age, gender, race, social or immigrant status, sexual orientation or religious affiliation. SSM provides emergency medical care regardless of a person's ability to pay.

Eligibility

To be eligible for financial assistance, you must complete a Financial Assistance Application which is also available in Spanish or by contacting a Customer Service representative.

Your application should include the documention below.  Download a checklist in English or Spanish.

  • Checking & savings account statements (last three months)
  • Verification of income (last two months)
  • Last year's federal tax return or non-filing letter
  • Verification of monthly expenses
  • And, Medicaid or Medicare denial/approval

For more information about financial assistance, contact an SSM Health Care Customer Service Representative toll free at (855) 989-6789, Monday through Friday, 8 a.m. - 6:30 p.m. CST.

A Customer Service representative can also provide a copy of our Billing and Collections Policy which describes the actions that SSM may take in the event of nonpayment and is provided for free upon request.
